Please note you will be prompted at
Clock-out to indicate if there was a change of condition or a fall. Select “Yes” if there was.
If you select yes, please ensure you call and report this immediately if you have not already done so.
Please also call if you are unsure if a fall was reported, or if they fell while our staff was not with them.
During a fall, ask yourself:
Is assistance available nearby?
Is immediate medical intervention needed? If so, call 911.
Is it safe to assist the client up?
Can safe transfer techniques be used to help the client from the floor?

This is where you:

Enter mileage (only when driving a client in your car or running errands for them in your car)

You are trying to clock in/out too early or late
Your agency may have a clock in/out window set which will not let you clock in/out unless you are so many minutes away from your shift’s start/end time. Check with your agency to see what your clock in/out window is set to.
You are trying to clock in/out too far from the home
Make sure you are inside the home when you attempt to clock in/out.
You do not have a visit scheduled.
If you think there may be a scheduling mistake, please contact your agency to ensure that you are in fact scheduled for a shift
You have not completed your Activities of Daily Living
Your agency may require you to check your ADLs off + enter visit notes before clocking out. If this is the case, be sure to mark all activities as either complete or incomplete before attempting to clock out.

To request additional training, please call the office and ask to speak with your case manager or the Client Services Manager.

We always strive to provide excellent customer service and go above and beyond for our clients whenever it is safe and appropriate to do so. If a client requests something that is not listed on the Care Plan, use your best judgment to determine whether it can be completed safely and falls within the normal duties of a caregiver or CNA.
If you are unsure whether a task is appropriate or you are uncomfortable performing it, contact the office or your Case Manager before completing it. If a client continues to request a task that you have been instructed not to perform, politely explain that you are unable to complete it and encourage them to contact the office if they have any questions or would like to discuss updating their Care Plan.

Open the AxisCare Mobile App and log in.
Tap Schedule and select your current client or upcoming shift.
Once the shift opens, tap forms button to view and sign the care plan.

Simply click the three dots to the right in your Axiscare app and login. No username needed. Please complete initial training within 30 days.

Weekend Differential (+$2.00)
Begins for shifts that start after 5pm on Fridays.
Stops after shifts beginning after midnight on Sunday.
Onboarding Rate is $10 / Hr

Request your days off with your case manager giving advance notice
Once your day off is approved, log in to Paylocity and request your PTO hours in the app
Choose request type as PTO if you are looking for Paid Time Off
Choose date you would like PTO
Choose how many hours you want to use
Submit your PTO request
If approved, you will have PTO for the day requested

Under the Fair Labor Standards Act, Home Care workers are considered “Engaged to Wait". This means you cannot leave your clients unattended for breaks and meals.
You will be paid for your entire shift, including the times you are eating or sitting with your client waiting to assist them.

Hair and scrubs are kept clean and neat.
Only natural hair colors may be worn.
Perfumes and colognes should be kept to a minimum. Many of our clients are very sensitive to fragrances.
Name Tags must be worn at all times while you are on duty.
Tattoos Covered, Piercings should be replaced with spacers
Nails should be short and painted with a neutral color
Shoes should be in good repair. They must be close-toed and non-skid soles
Scrub Colors: Black, Emerald Green or Navy Blue

To stay active with Overture, you must work a minimum of 4 shifts a month (once a week)
If you go inactive, you cannot return to work until 6 months after your deactivation date

You must not go a 30 day period without working or we will accept that as your resignation
You must actively request Open Visits in the Axiscare app, if you do not request any in 30 days and are not working we will accept that as your resignation
If you decline or call out of shifts after requesting them, that does not count as having worked or requested shifts

Office Hours: 9a-5p M-F
Please call during office hours for anything related to Payroll, HR, scheduling requests, or general questions
After 5pm and Weekends:
Answering Service will take a message and relay to office staff on duty
Only call after hours to report falls, client emergencies, or client questions, or if you cannot make your shift

Excused:
With 12+ Hours Notice
With a Doctor’s Note provided within 24 hours of returning to work
Proper Documentation for any other emergencies sent to your supervisor
Unexcused:
Less than 12 Hour Notice
Notifying office via text or email (You must Call)
Two + unexcused absences within a 30 day period will result in:
1st – Verbal Warning
2nd – Written Warning
3rd – Termination
Regarding Tardiness, your first 3 do not count against you. Once you have your fourth in a 6 month period, it will result in:
4th Tardy - Verbal Warning
5th Tardy - Written Warning
6th Tardy - Termination
What calls for termination?
Calling in 3 times in 30 days
Calling in 4 times in a 6 month period
Calling in unexcused 8 times in a year
Calling into your first shift
Not arriving to a shift without calling / or calling out after your shift was to start (No Call / No Show)

If found sleeping, the following actions could occur:
If intentionally sleeping, you willbe terminated
If unintentional and you do notinform us, you will be terminated
If a client is injured due to you being asleep, the State of Texas may revoke your CNA certification, list you on theEmployee Misconduct Registry, and you will be terminated from employment
While on shift, do not:
Lay down on a couch or the floor
“Rest” your eyes
Cover up in a blanket or use a pillow
Prop your feet on any client furniture
Slouch where it could appear you are asleep
Clients may report these behaviors, and if it appears that you are sleeping while on duty, it may result in disciplinary action up to and including termination of employment

Contact your supervisor immediately
If it is an emergency, leave the home and call 911
You are welcome to report on your own to AdultProtective Services (800)-252-5400

What to do if you notice a change?
First notify the facility or Hospice company if applicable
Second notify Overture Home Care
Then contact the clients POA or family member

Falls:
If they are unconscious immediately call 9-1-1 (unless on Hospice)
Caregivers DO NOT PERFORM CPR
If at a facility, contact the facility staff
Then contact Overture Home Care
Next contact the POA or family
Vial of Life: contains client's medical history, medication list, allergies,and emergency contacts (Vial of Life may be found on the back of thedoor or in the refrigerator, location may be listed in POC)
